
What Insurances Do We Work With?
Authorizations and Payment
To be seen at Restore Physical Therapy, we do not require that you have a referral from a physician. However, some insurances do require a referral—some of the most common being Sunflower, Tricare, some United plans, Kansas Building Trades, Highmark, VA, and Workers Compensation. We do our best to expedite your authorization to maintain the progress of your treatment.
We do not require payment up front for each visit. However, if we find that you have a copay, you are welcome to pay that with each visit. Upon your first appointment, we will pull up your insurance coverage and place that information in your patient folder, along with a ballpark estimate of your per-visit pricing. Don’t hesitate to ask questions about the billing process.

We have minimal paperwork. We ask that you either show up to your first appointment 5 minutes early, or, if you’re able, fill out your paperwork ahead of time by following the links below.
