Preparing for Your

First Session


We have minimal paperwork. We appreciate it if you arrive 5 minutes early. Please bring a physical copy of your insurance card, worker’s compensation claim number, or case manager’s contact information, depending on your payment method. If you were involved in an accident, please bring the claim number and agent contact information.


If you wish to fill out paperwork ahead of time, please refer to the “Insurance” tab for the option to print our forms.


Please wear loose and comfortable clothing. Expect to spend about 45 minutes with us.

Your First Session


During your initial evaluation, you and your physical therapist will discuss your medical history, assess mobility and coordination, and analyze muscle strength and gait mechanics. Using this information, your therapist will design a personalized treatment plan for you, including the recommended frequency and duration of therapy sessions going forward.

Restore PT is a terrific clinic. I’ve known few places where every single staff member is so wholeheartedly committed to taking such great care of their customers.
